Evo's Weight Loss clinical protocol is available to all Evo members.
IMPORTANT: Prescriptions for GLP-1 medications are only provided when clinically appropriate, as determined by your provider. If you are considered an eligible candidate after your first visit, lab work will be required. The provider cannot prescribe generic or compounded versions of GLP-1s or complete prior authorizations for prescription benefits. Retail prices for brand-name GLP-1s may be very high, so we recommend contacting your insurance company in advance to understand your prescription coverage for GLP-1s.
It is always up to your provider's discretion to determine if your condition can be addressed with Evo.
