To view your doctor's note:
- Log in to your account.
- Go to your Profile and Visit History.
- Open the Treatment Plan for your most recent visit.
- At the very bottom you will see an area to download your doctor's/school note.
- If you do not see this, it means your clinician did not provide a doctor's/school note. You may request a note by following the next steps.
- Scroll down until you see "Visit Follow-Up" and click "SEND PROVIDER A MESSAGE".
Note: There will be a message under Visit follow-up that will tell you how long this option will be available. - A pop-up will open. Leave your message asking for a note, and leave your phone number. Click "SEND MESSAGE". This will notify the provider that they have a message.
- When the provider responds a notification will be sent to you via email or text message.
- To view the note: Sign into your account and go to "Profile and History". Click "Visit History", locate the visit, and click "View details".
